Very informative . I have a friend out in the Philippines sadly healthcare there is not the best and I really don't think they are telling her the truth on everything she has been battling breast cancer for 4 years she has pretty much kicked the cancer behind but sadly it has done damage to her bones now she has compression fractures and lumbosacral in APL projections show diminished height of L2 and slight left curvature of the lumbar spinal column. What would you suggest our next steps would be , we have done x-rays and do MRI next week she has been in severe pain for several weeks and no longer can stand or walk without help. Any input would be greatly appreciated
Thank you for your comment. Treatment would depend upon the results of the MRI studies and x-rays. It would be difficult to surmise the next step without the results of those studies. It certainly is good news that she has gone into remission.

I just had this procedure done yesterday Dec 1 2022,it's the next morning and the pain isn't as bad, but hasn't left as of yet. When I stand up it gets really tight in my back, but I'm guessing this is normal. I suffered a crushed T12 vertebrae just from opening my window. Can anyone tell me if the pain goes away any time soon?

Thank you for the comment. Most anterior compression fractures of L1 are not unstable and are typically treated with a brace and mobilization. I would have to have more information about the type of L1 compression fracture to make a comment. If the surgeon thought it was unstable to begin with then follow up studies will be done to determine if it is still unstable.
I'm based over in the UK and am looking at needing this surgery at some point. Had problems for over 20 years which are now progressing. Thank you for the education so I know what to ask my NHS surgeon (who likely will not be using these modern techniques) so I can choose whether I have to go private for better treatment.

Why would I need a fusion if my Grade One Spondylolisthesis never shifts but does move when standing up or laying down? Some docs say I need a fusion, and some say I don't. Very confusing.
Remember Tiger Woods. He had a micro discectomy several times. Finally he had a fusion and within a few months won the masters. The chance of good results with a fusion is 90% plus. Some doctors will offer to do surgery without a fusion. I believe they’re playing on the misconceptions of the public. Most of those doctors that do a decompression know that within a year or two you will have a fusion. They get two surgeries. The recovery time and the postoperative pain with a decompression and no fusion is much longer. It’s counterintuitive to most patients. Fusion‘s got a bad name in the lumbar spine because fusions were being done for degenerative disc disease. Patient satisfaction for a lumbar fusion for degenerative disc disease is very low. That does not mean all fusions are bad. Cervical fusions for example have a 96% excellent result.
I really appreciate your video. Very INTERESTING, to say the least. I have a question or three for you, doctor. In March 2016 I had a 3-level fusion cage surgery on S-1/L-5 - L5/L4 - L-4/L-3. The disc material between L-3/L-2 was about 50% gone and the ortho surgeon elected to NOT put an artificial disc in that segment. Rather - extend the rods and screws higher to keep pressure off the disc. Doctor used two rods - two plates and screws to hold everything in place outside my spine. Surgeon ground down facets and used the powder of them to be part of my inserted discs, if not all. Don't remember now. Doctor told me he felt he could give me about 70% OVERALL relief of my back pain prior to surgery. That percentage was considerably higher than what 5 other very prominent ortho surgeons promised me in the Grand Rapids, Michigan area. (University of Michigan refused to do surgery on me, as did the 5 local surgeons). There was considerable relief from back pain for about 1 1/2 years. Then slowly it started increasing. After three years I went back to the surgeon who did my operation and asked him if there was anything he could find to help reduce my pain. He told me the MRI and x-rays he took of the area showed everything was in place as it should be. "So there's nothing I can do for you ... other than recommend you try some sort of an implant that sends electrical signals to the spine area." (I forget the exact name of the procedure, [not a Tens unit; but similar in purpose] but it didn't look all that attractive to me, and so I've never seen him again). Doctor told me what I was experiencing was S-I joint pain. (Little more about that below). Now what I find VERY INTERESTING to me about your video here is that my surgeon said NOTHING about the possibility of having my rods and screws and plates one day removed, IF I qualified. NOTHING was said about it, so I just assumed the hardware was intended to stay inside me until I died. But YOU indicate maybe NOT so???? Although I don't have discs moving in and out constantly like I had before my surgery, my back pain has consistently began getting worse over the last three years (six years after the surgery). I can only stand on my feet for about 30 minutes and my legs get so heavy I have to lie down. Yep - all the numbness and weakness in the legs, but that hardly left after the surgery). I've read extensively on Doctor Google that with as many discs that I had fused, especially the L-5/S-1, it's normal for trauma to move down into the S-I joints to act as the new "shock absorbers" I had one S-I injection from the side that radiates the most pain and it worked quite well for a couple of months. Then wore off. I had two more in the same area and I left the clinic with more pain than what I went in with. Decide NO MORE injections. So now comes my question/s. Is it possible that if I had my hardware removed, might I receive a lot of freedom from the back pain I live with now day and night? And would you recommend I NOT go back to the same surgeon who said NOTHING to me about the possibility of one day having the hardware removed, because I neither really would need it ... or it might be causing me undue pain? He is a highly rated spine surgeon according to many I spoke with, but personal recommendations can be quite "relative" I realize. Thanks for any suggestions you might care to offer. AND - if you have an ortho surgeon in the Grand Rapids, Michigan area you might recommend in checking me out, I would surely appreciate it. I would be willing to travel further if convinced it would be worth my time.

The most common reasons you would have pain after a long fusion include: assuming that there is no problem with the fusion itself and it is a solid fusion with no residual impingement. 1. Breakdown above the fusion which is typically treated with a fusion and decompression. 2. Breakdown of the SI joints. Usually diagnosed with x-ray CT and bone scan. Treated with a fusion of the SI joints 3. Symptomatic hardware. Typically treated with removal of the hardware. 4. Post laminectomy/fusion syndrome. Pathology from previous surgery not correctable with additional surgery. Typically treated with pain management and devices such as dorsal column stimulators. Without having access to your studies it would be impossible to know into which category you fall. If everything else is negative and your pain is mostly in the lower back over the instrumentation then you might be a candidate for metal removal. Normally I would think it best to go back to the original surgeon. Just be aware of the fact that the original surgeon is always going to be most interested in proving that his original surgery was done well and not necessarily look for secondary causes. I'm on medicare, medicaid here in Ohio and am facing this surgery but they want to slice me open with a huge incision and cut into tissue at Riverside Hospital. no thanks. Thanks for your kind comments. Yes. The bone kyphoplasty can be used in a disc. That would convert the disc to bone and result in a fusion.
Do you suggest Bracing after Lumbar Fusion and bed rest or walking with no Brace? I had a failed fusion we assume I have micro movements so in pain when bending. It has been 2 years since fusion, I should be going for revision in about a 2 months.

I had L4/L5 fusion from the back, 2 years ago this April after a re rupture of the disc after a discectomy a year and 1/2 before that, and although all the sciatic pain is gone, when I do anything where I bend over or my spine moves I get shocking pains in my back. Rest usually gets me back to pain free. Just sitting and shifting my hips front to back, side to side I get pain. If I lay on my back I am usually pain free and can sleep in most positions, sometimes laying on my stomach will cause pain. The CT is said to be vague as to whether I got enough bone growth. After the last Fusion I was off all meds and tylenol day 3, and was walking day 2. I am hoping the revision will fix the pain so just looking at doing whatever I need too to get that bone to grow. Non diabetic non smoker, non drinker and not over weight. Don't take supplements or use pain meds even OTC unless I really need them. Any advice would be great. For the Revision surgeon said he would probably go in again from the back remove the old hardware, use bigger screws and I assume use another cage and bone graft.
I recommend avoiding braces and normal activity. Braces have shown that biomechanically they increase the amount of stress on the lumbar spine. Also Braces rest the muscles in the spine and lead to muscle atrophy with a net result of more stress on the spine. Bones like activity and stress according to Wolffs law. Do you want to avoid stress on the bones. Direct compression is good. You want to avoid sheer and twisting. No brace. Normal activities. Minimal lifting and no lifting and twisting. Want to use good body mechanics and keep your shoulders and pelvis in the same line.
I have 4 thoracic wedge type compression fractures from Multiple Myeloma (2013) due to my job as a San Antonio firefighter. 62 yr old retired upon diagnosis. Tried everything. Opioids, ablation, braces, heat, lidocaine patches etc. Physical therapy/exercise makes it worse. 3 pains specialists. No relief. Can't sleep, function. Life is miserable. Kyphoplasty was never even offered at diagnosis. Is this an option? Currently covered by workers compensation.
Kyphoplasty only works in subacute situations. Normally it should be done in the first six weeks. Occasionally patients will benefit after six weeks. Once the fracture has healed then a kyphoplasty does not work. Would be happy to review your studies.

If the fracture is 19 weeks old it is unlikely that any height will be obtained using a kyphoplasty. There is an exception if it is a fracture where there is fluid inside the vertebral body. Some fractures have a fluid line across the top or bottom of the bone. Those type of fractures even late can have some height restored.
Yes. If a kyphoplasty is done before the bone starts to heal the height of the compressed bone can be restored.

Five months is a long time to still have pain. Her first step would be to see her primary care physician and have some studies done.
I had this done today.. so happy I was asleep. I am in a bit of pain, can’t sleep. Hoping for the best. However they suspect cancer in my case.

Makes me sad that I suffered a work place injury with a compressed fracture in 2018 and no one recommended this to me, 3 years on and I suffer with back pain every day... is it too late for me to undergo this procedure?
I suffered 7 months with a compression fracture and it then crushed and I had this procedure done yesterday Dec 1 2022. The surgeon did tell me that since it had been 7 months before having this done,I had a 50 to 60 % chance of it actually working or helping me,but I'm very hopeful and the procedure went well and no problems and no bone cement leakage,I would talk to a surgeon about this.
Most likely it would be too late for this exact procedure. It would certainly be worthwhile having some studies and determining if anything can be done at this time.It’s never too late don’t give up hope.

Plz help me out to figure out with some concluctions: (1): can my pars defect can be heal with proper treatment ,bracing and medication.. since im a bit late to diagnosed with ingury(after 1&half months) (2)Im 32...im not sure if this could be easy if pars defect could heals in this age!!! Pars defects can heal if they are acute fractures. A CT scan is usually done to check the healing potential and follow the healing. That is a good question. I may do a video on that topic. A young football lineman or a gymnast can fracture the pars and it can heal.

The skin is coated In an antibiotic impregnated plastic. Has been shown to keep the level of infection down.
Did a lot of Live surgers for universities in Texas. Respectfully some advice, dont use to many cameras(confusion to some views). Best views is POV, Directly over the procedure and instrument equipment camera. If you have a hard time to convert analog output from your standard medical equipment camera to digital. I can give you some products that can help.
Thank you for your input. A POV surgery is good for surgeons but the general public goes to sleep/finds them boring. We’re always experimenting with different techniques. Always looking for more ways to make the videos more entertaining.
Thanks a lot! I had surgery about 3 weeks ago, my Doctor didn't tell me those information 😞I felt too afraid. And still I have more numbness and week left leg after the surgery! Surgery is always scary. I am always trying to develop better ways of informing the patient but I realize surgery is never something people will look forward to, ha ha. When you have surgery on the sciatic nerve/nerves they manifest numbness and tingling in the legs at least for a few weeks. Most likely your numbness is a normal finding in the type of surgery that you had. You should discuss your concerns with your surgeon when you see them again. An MRI would most likely not be helpful at this point because numbness is probably a normal finding. That’s another good question for your surgeon. Thanks for the comment. I am 23 years old and I already have 3 compression fractures on my back. I sadly compressed fractured my T12 similar to the one in the video. Although, mine is a bit more compressed. A neurosurgeon has recommended this procedure for me. The fracture happened in 2016 and I do still feel back pain. My T11 & T12 are so close to touching and my nerves are always spasming. Is this procedure something that will truly helped me in the long run? Has anyone had this surgery and can speak from experience? I am trying to get feedback before I decide to do a surgery on my spine.
A kyphoplasty is used on a fracture before it heals. It would be too late if the fracture occurred in 2016. If a bone spur is touching a nerve the surgery would be a laminectomy. You should probably talk to your surgeon again.
It is interesting how the prefabricated disc was never questioned. This prefabricated disc is hard. There is no give to it. You are replacing something that has none of the qualities of a human Natural healthy disc. It would seem that this prefabricated disc would be painful post surgery.
A prefabricated disc is used as scaffolding for bone growth to achieve a fusion.
Hello Doctor. I am 37 male. 2 months ago I was diagnosed with spondylolysis L4. Symptoms are buttock and lower back pain. Is it possible to completely heal from the spondylolysis L4 (stress fracture recovery) at my age by doing proper stretching and gaining abdominal muscles on daily basis?
If it is a stress fracture spondylolysis then it can heal. By age 37 it could be a nonunion which is unlikely to heal. Would need a CT scan. L4 spondylolysis is less likely to heal than an L5.
Why did you choose stitches over surgical glue, won’t that leave a pretty ugly scar? I’ve seen this done where they used surgical glue on the outer layers of skin. That just looks like Dr. Frankenstein got ahold of the person’s neck..
We use subcuticular suture with glue on skin. No suture in skin.

I wonder why the doctor don't use a videoscope to see inside the spine when he removed the escess bone and tissue in the first half of the surgery. After all, the area is very small and dangerously close to the spine and nerve.
